Everything You Need Before Your First Appointment

Our periodontists and periodontal team members are grateful that you’re considering us for your periodontal needs. Before you schedule an appointment, though, we advise that you take the time to review and consider the following information. This will help you prepare for your upcoming appointment, so you aren’t caught by surprise at any point. Both of our periodontal offices are just a phone call away, so please reach out if there’s something on these pages that you don’t understand.

Your First Visit

Your first visit is likely to be a consultation where we examine your mouth, take any necessary X-rays, and explain your treatment options before setting up future appointments. Before we begin, you’ll need to complete any paperwork that you haven’t already finished; our front desk team will be there to help if necessary. Once your visit has begun, you can expect us to ask a series of questions regarding your symptoms, your overall health, and your goals for treatment. Remember that you can always ask questions and that treatment will only move forward once we’re sure that you fully understand the situation.

Periodontal Patient Forms

When we meet patients for the first time, our top priority is to gather information about their current oral and overall health. After all, there’s no such thing as a “one size fits all” periodontal treatment; we need to personalize your treatment plan based on your needs. As such, you’ll need to fill out a series of forms before your appointment. Our team can answer any questions you have if you’re unclear on the instructions.

Dental Insurance

We patriciate with Delta Dental & Anthem insurance. We submit to all other insurances providing their are out of network benefits. Our team will handle the paperwork end of things, so you can rest assured that the right forms will be filed to let you take full advantage of your benefits.

Dental Financing

If you don’t currently have dental insurance, there’s another way to pay for expensive treatments without breaking your budget: third-party financing. A third party will cover the costs of your care, then let you pay it back over several months via a payment plan that has been carefully chosen according to your budget needs. Depending on what plan you select, there may not even be any interest to worry about. Call today to learn what financing options are available at our practice.
